Notice on the Launch of Electronic Health Declaration Form

Starting from 13 November,2020, all foreign nationals who are flying from New Zealand to China will be required to present the Electronic Health Declaration Form for boarding. Foreign nationals can apply for the Electronic Health Declaration Form by logging on the website of https://hrhk.cs.mfa.gov.cn/H5/ via computer or smart phone. Foreign nationals can fill in the information, declare the health status and upload the testing reports online, which will then be examined and verified by the Chinese Embassy or Consulate-Generals in New Zealand. Upon approval, you will receive the Electronic Health Declaration Form, which will be displayed in QR code with "HDC" mark. Foreign nationals must present the electronic or printed code, and follow the procedures for inspection by relevant airlines during boarding.

Chinese nationals (including the Hong Kong SAR, the Macao SAR and the Taiwan region) can still apply for the green health code with "HS" mark via the International Health QR code in the WeChat mini program.
