An Update on the Requirement for Requesting Health Code (Health Declaration Form)

It has been occurring that incomplete or incorrect information were provided when people requested the Health Codes (Health Declaration Forms) since the new requirement took effect on 1 March 2021. In order to avoid any delay in granting the Health Codes (Health Declaration Forms), please be reminded of the following:

1. Chinese nationals and foreign nationals shall use different platforms for application.

1) For the Chinese nationals (including Hong Kong SAR, Macau SAR and Taiwan) to apply for the Health Codes, please use the Health QR Code (international version) in the Wechat mini program.

2) For New Zealand citizens and other foreign nationals to apply for the Health Declaration Forms, please access https://hrhk.cs.mfa.gov.cn/H5/.

2. The required documents should be uploaded in full at the time of requesting the Health Codes (Health Declaration Forms).

1) General documents for requesting the Health Codes (Health Declaration Forms):

a. The original COVID PCR test report of negative outcome as issued by the laboratory, instead of any GPs or clinics, within 2 days prior to the scheduled flight departure;

b.The original COVID IgM serology antibody test report of negative outcome as issued by the laboratory, instead of any GPs or clinics, within 2 days prior to the scheduled flight departure;

c. A vaccination certificate for COVID-19, for those who get a positive outcome of COVID serology antibody test after receiving a COVID -19 vaccine.

d. Electronic ticket or flight itinerary bearing the applicants’ full names and detailed flight information and issued by airline companies or travel agents.

2) Visa or other proof of residence based on the applicants nationalities.

a. For the Chinese nationals (including Hong Kong SAR, Macau SAR and Taiwan), please upload your New Zealand visas or resident permits. For the Hong Kong and Macao residents, please upload your Mainland Travel Permits. For the Taiwan residents, please upload your Mainland Travel Permits.

b. For the New Zealand citizens, please upload the information pages of your valid passports and valid Chinese visas (or Chinese residence permits).

c.For other foreign nationals, please upload the information pages of your valid passports, and valid Chinese visas and New Zealand visas or residence permits.

3. The passengers are kindly requested to set up enough time to apply for the Health Codes (Health Declaration Forms) and make sure to retrieve these documents before heading to airport.

To avoid any unnecessary last-minute hassles, please make sure that you have enough time to obtain the Health Codes (Health Declaration Forms), as you may need to upload a large number of pictures which may be very time-consuming with low internet speed. It is highly recommended that the above-mentioned documents be uploaded through the proper platforms as soon as the necessary test reports are available.

Please make sure that all your personal details are correct, accurate and consistent with the test reports and that the whole set of documents are uploaded.
